ACT Nurse Reports to: ACT Team Leader Position Summary: The ACT Nurse is a core member of The Well’s Assertive Community Treatment (ACT) team, providing nursing care and integrated support to adults with serious mental illness (SMI).
This role manages the medication system, administers and documents treatment, monitors health conditions and side effects, coordinates with outside providers, and promotes health education and prevention.
