Care Coordinator FPP

Maintain the department’s master patient list and/or tracking systems, including the timely addition of newly identified patients to support appropriate clinical follow-up, care coordination, and continuity of care.

Ensure patients requiring additional review or higher levels of clinical management are appropriately identified and reflected on applicable tracking lists.
