Care Coordinator

Job overview We are looking for motivated and enthusiastic care coordinator to work in the Working Age Community Mental Health Service, support service users recovery within the community.

You will be recovery focussed, have good risk assessment skills and have a good range of therapeutic interventions supported by evidence-based practice.
