Description

Purpose: To provide engagement and case management support, including home-based visitations and other services within the Philadelphia community.

Interact with members of the multidisciplinary team to ensure that participants receive the individualized services designed to help them move towards long-term housing.

To provide services in compliance with contract and Agency regulations.
