Description
Overview Case Managers provide two services.
One is is Adult Mental Health Case Management and the other is Behavioral Health Home Services.
We call these workers Integrated Care Managers.
Integrated Care Managers advocate for, facilitate, and coordinate services that a person needs to pursue their goal of recovery.
They are a central point of contact to ensure that people are receiving integrated care and services recommended for them that supports their personal recovery vision.
