Job Description Job Description Responsible for conducting risk/needs assessments, developing and implementing service plans for all clients assigned to their caseload, facilitating scheduled workshops/programs/groups, and developing discharge plans.
Scope of Duties: Conduct orientation and assessment of all new clients via the approved agency Assessment Tools.
Complete Service Plans on all assigned clients based on approved agency assessment tools with client input.
