Description
ROLE PURPOSE This position will provide outreach support to client(s) referred to Connective by Community Living British Columbia.
You will work to enable clients’ adjustment to and success with life in the community.
This is a person-centered role where you will go out into the community to meet clients and develop individualized goal plans which can include accessing community services, housing and employment supports & life skills development.
