Social Worker – Wrexham Children Services Leaving Care Team.
Social Work qualification must be held.
The Leaving Care Team work with care experienced children and young people aged between 15-25.
The focus is to support with transition planning through to independence.
You will be committed to corporate parenting, relationships-based practice as the driver of positive outcomes and transitions.
