Community Care Nurse

POSITION SUMMARY Reporting directly to the Health Manager, the Community Care Nurse is responsible for assisting in the implementation and evaluation of the home & community care, Adult In-Home Care, Diabetes, Chronic Disease Program, and Community Health Program.
