Description
As an Adult Social Worker within the South Community Team, you will manage a complex caseload of adults with care and support needs, completing assessments, reviews, and support planning in line with relevant legislation, including the Care Act 2014, Mental Capacity Act 2007.
You will work closely with individuals, families, carers, health colleagues, and partner agencies to develop personalised support plans that promote independence, wellbeing, and positive outcomes.
