De functieAls HBO verpleegkundige chronische verslavingszorg houd je je bezig met patiënten behorende tot de Openbare GGZ (OGGZ)-doelgroep en met afstand tot de reguliere zorg.
Je bent caseloadhouder van een aantal patiënten en verantwoordelijk voor de coördinatie van hun behandeling.
Je benadert actief zorgmijdende patiënten, draagt bij aan behandelplannen en werkt als medebehandelaar in een multidisciplinair team.Je wordt ook ingezet als omloopverpleegkundige.
Je voert triage uit bij somatisch
