Home First Facilitator

Job overview This is an exciting and new opportunity to work as part of the Integrated Discharge Team, supporting the teams to implement the Hospital Discharge policy with the 'Home First' ethos.

This role involves working as part of a multi-disciplinary team to ensure patients are discharged from acute care in a timely manner, reducing delays and supporting people to return to their own home or familiar place whenever possible.
