Maternal Health Nurse Navigator

In response to identified needs, the Maternal and Child Nurse Leader will reduce local disparities in maternal and infant health outcomes, through education, screenings and community resource referral and clinical care.

This position will have a focus on coordination of resources between inpatient obstetrics, outpatient maternal healthcare, and maternal community health improvement.
