Description
Community Health Centers of Burlington is seeking a Medical Respite Social Worker (MRSW) to be a liaison between the hospital and the medical community for all referrals to, and placements in, the medical respite program.
The MRSW will work with hospital staff, as well as other referrals sources to ensure safe, appropriate medical respite placements for optimal patient recovery and with the goal of avoiding the need for (re)hospitalization.
