Job Description Job Description JOB PURPOSE: Conducts re-entry intakes, coordinates release planning, collaborates with Re-Entry Case Managers, and ensures continuity of care for individuals receiving Medication Assisted Treatment (MAT).
Serves as the primary non-clinical liaison between Re-Entry, the MAT RN/Medical Staff, case managers, and community-based treatment providers.
