POSITION PURPOSE: Coordinate referral authorization process between the provider and the insurance plans.

Initiate preventive and Health Risk Assessment, Assisting clients with request for transfer to CHI as their Primary Care Provider.

Provide Clients with general information regarding benefit coverage.
