Description

This position is currently in Grande Prairie.

You will be working in conjunction with our LPNs as well as work with HCAs that are supervised by the Personal Support Coordinators.

Additionally, you will work with Allied Health, community paramedics, EMS, lodge & other community partners.

You will be required to work collaboratively within a multi-disciplinary team as well as with the clients and their support networks.
