Registered Nurse - Community Home Health Liaison

Job Description Shift : Days Status : Part Time; 24 hours per week GENERAL SUMMARY: The RN Community Home Health Liaison acts as a liaison between Allegiance Home Care Services, the hospital, physician's office, and other community service agencies to manage and facilitate the safe and timely transfer of patients to the home care setting, for patients who have elected to use the services of Allegiance Home Care.
