Senior Case Worker (Continuing Healthcare)

As a Senior Case Worker in the Hampshire County Council Continuing Healthcare (CHC) team, you’ll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC.
