Description
Pay rate – £25 per hour Contract – Temporary Liverpool City Council are currently recruiting experienced Social Care Assessors for their Hospital Discharge team based in the Royal Liverpool Hospital.
35 hours per week Newly Qualified Social Workers will be considered, with a view to offering longer-term opportunities.
Working within a multi-disciplinary NHS and local authority network to plan safe, timely, and person-centered transitions from hospital to home or interim care.
