Social Worker

The Grade 8 Social Worker is based within the Community Adults Team in Crewe and supports adults aged 18 and over with care and support needs.

The role sits within Adult Social Care and involves undertaking strengths-based assessments, safeguarding enquiries, care planning, and reviewing support arrangements to promote independence, wellbeing, and positive outcomes.
