Targeted Case Manager (55255)

Description POSITION SUMMARY: The Targeted Case Manager (TCM) will provide case management services to individuals in their assigned program of severe mental illness (SMI), substance use (SUD), and/or serious emotional disturbance (SED).

Position consists of resource acquisition, linking to services, coordinating care for physical and mental health needs and monitoring changing needs of participant.
