Urgent Community Response Occupational Therapist/Physiotherapist

Job overview The post is for an Urgent Community Response therapist who will have extensive clinical skills required to help support people in their usual place of residence with an urgent healthcare need (within 2 hours) which will involve an assessment and short-term interventions.

The post holder will also work in partnership with 111, 999, community teams, care homes, social care staff and the wider multidisciplinary team to provide appropriate patient focused care.
